Submit an ID Request Form Online
You need the following to proceed
- Have, or be ready to take, a digital photo meeting our Photo Guidelines
- Have your Government-issued photo ID in hand or readily available
- Have your Banner ID (V#) written down or readily available
- Know or claim your eID and password using your Birthday and Banner ID#
How this process works
- Have the above list prepared or you will be timed out of the application
- Step 1: You will complete an ID Request Form by clicking the link below
- Step 2: After completing Step 1, the submit button will automatically redirect you to upload your photo and copy of your government-issued ID
What you should know
- This online process is for first-time cardholders; returning students should visit our office if they no longer possess their most recent VCUCard
- You can only complete one ID Request Form
- Instructions for completing Step 2 are automatically sent to the email addresses you provide on your form
- Once you complete Step 2, you must wait for an email from the processing team confirming the finalization of your ID Request Form and the printing of your VCUCard
- Orientation requests take priority, and are processed in order of orientation date; all other requests (including pick-up and mail) are processed last and in order of receipt
- VCU Health Employees requesting a student VCUCard must schedule an appointment at our office on the MCV Campus and bring their current VCU Health Card(s) to their appointment; do not complete this online process
- If your orientation deadline has already passed, and you did not complete this process in time, you will need to visit our office to get a card
