VCU Health Cards
The following information pertains to members of the VCU Health community who are obtaining a VCU Health Card. Use of the VCU Health Card by the cardholder signifies agreement with all Campus Card Terms and Conditions provided by the VCUCard Office, and agreement with all terms and conditions provided by the VCU Health System HR Identification Card Policy.

The Requirements
To obtain a VCU Health Card, you must:
- Have an eligible active status
- Provide verification of your identity
- Provide your photo when requested
Getting Your First VCU Health Card
VCU Health Employees are health service team members who are employed either by the VCU Health System, VCU Health MCV Physicians, or VCU Health CMH Physicians. Employees are provided information from the VCU Health System about how to receive their VCU Health Card. Typically, ID photos are taken at the VCU Health HR Express office when completing paperwork, and employees will receive their card at the time of their scheduled orientation.
If you are not scheduled for an orientation, please schedule an appointment.
VCU Health Affiliates are individuals who are affiliated with but are not paid by VCU Health, which includes contingent workers, contractors, and volunteers who provide hospital-related services. All affiliates should request a VCU Health Card in person.
All affiliate first-issuance and replacement cards cost $30.00. First-issuance cards are typically billed to the sponsoring department.
Get Carded
In-Person: Before scheduling an appointment, please call us to verify the affiliate account status. Then schedule an appointment to visit our office on the MCV Campus. We take your picture and print your card. Appointments usually take less than 10 minutes.
VCU Health Sciences Students are enrolled in master health-related curriculums and are typically in one of five health sciences schools: College of Health Professions, School of Dentistry, School of Medicine, School of Nursing, and School of Pharmacy. All Health Sciences Students should request a VCUCard in person at our office located on the MCV Campus.
In addition to a VCUCard, Health Sciences Students can request a secondary VCU Health Card. The VCUCard is the primary Campus Card and will house all access privileges. Students are responsible for displaying the appropriate card for the role they are in the process of performing. Request a VCU Health Card in person at our office located on the MCV Campus.
School of Nursing: VCU Health Employees who enroll as students in the School of Nursing will surrender their current VCU Health Card in exchange for 2 new cards: a VCUCard (the primary card that houses all access privileges) and a VCU Health Card (the secondary card for health-related identification).
Get Carded
In-Person: Schedule an appointment to visit our office on the MCV Campus during business hours. We take your picture and print your card. The process usually takes less than 10 minutes.
Replacing Your VCU Health Card
Only the first issuance of an employee card is free. There is a charge on all cards issued after the first card, even if you are returning to the VCU Health System. Card costs are listed here.
The VCU Health System requires a supervisor's completion and approval of an HR4U ID Replacement Card Request Form before requesting a new card. The VCUCard Office does not have access to this form. For additional information on this process, visit the HR4U Portal or contact the HR4U Service Center at (804) 628-4748. You can verify if we received the approval by contacting us. Then schedule an appointment on the MCV Campus.
Lost Cards
Your card is considered lost if it is misplaced and you no longer know its whereabouts. Due to building access and funding privileges, you must deactivate your card to prevent misuse. You will be responsible for all card use, whether or not you authorize such use.

- By calling either VCUCard Office
- Visit our office on the Monroe Park Campus to receive a new card

- Deactivate VCU Building Access (if applicable) by doing one of the following:

- Deactivate VCU Health System Building Access
