Request A Campus Card For Orientation
Submit an online request for a card to pick up at your orientation.
You must do this first...
1. Be ready to take a picture meeting our Photo Guidelines
2. Have your Government-issued photo ID readily available
3. Know/claim your eID and password using your Birthday and Banner ID#
4. Have your Banner ID (V#) written down in front of you
Get everything ready before you start to ensure the process is smooth and simple
Double check the Photo Guidelines; taking a good photo is essential to this process
It is best to complete all steps in this process now (both form and photo), so you will have your card on your first day
After submitting this form, you will be automatically directed to Step 2 to take a photo and upload your verification of identity. Please do not begin your ID Request Form without having your eID and password to begin Step 2.
